Amy has been busy living her life and studying medicine, however, her worst nightmare is about to come true as the part of her study she has been freaking out about is psychiatry as she never wanted to go into this field. This is due to her past as when she was young, she started seeing a little girl and hallucinating. The little girl was like a devil on her shoulders giving her bad advice. The last time she was in a psych ward was when she visited her old friend Jade and the last words Jade said to her was that it should be her in the ward. Now years later, Amy has to spend the night shift on Ward D, and to make matters worse, her old friend Jade happens to be a patient and she's doing the shift with her ex-boyfriend Cam. As the night goes on though, strange things are happening people are disappearing and even some are dying. What is going on and what will happen when one of the patients who is supposedly dangerous gets loose on the ward and then a power cut makes escaping no longer an option? Can Amy and some of the patients survive especially when there is a mad serial killer psychopath on the loose in a locked nighttime ward where no one can hear you scream except the padded wall rooms?
